The North Central District Health Department is accepting applications for a Senior Support Service Associate I for the Henry County Health Department. This position is full time 37.5 hours per week. The entry-level salary is $8.82 negotiable. General Function for this position: registering patients for various services and programs provided by the health department. Establishing medical records for the patients, scheduling initial and follow up appointments.

Minimum requirements are: A High School Diploma or GED and one year of moderately difficult administrative office environment preparing documents using a computer, operating a phone system to answer calls and referring to appropriate persons, computer operation, (such as WORD, Excel, Internet, e-mail), using office equipment, (fax, copier, calculator) maintaining office files or filing documents. Additional education (college, vocational school. etc.) in business education or medically related field may substitute for experience on a year for year basis.

JOB- MNT Nutritionist I

The North Central District Health Department is accepting applications for a full time MNT Nutritionist I.

General Duties include: This position serves under the close direction of the Nutrition Services Supervisor or Director of Nutrition Services. Responsibilities for this position include, but are not limited to; Perform work of moderate to complex nutrition case management of medically high risk individuals including physician prescribed dietary and nutrition regimens and intensive counseling (Medical Nutrition Therapy/MNT). Perform work of moderate to complex management in the application of nutrition judgment and skill. Participate in on the job training activities to deliver basic nutrition and MNT services (group and/or individual contact) based upon programmatic skills, competencies and requirements. Assess basic and/or MNT nutrition status of individuals and develops a basic and/or MNT nutrition plan appropriate for each client or client group according to programmatic criteria. Interpret current research regarding basic nutrition and MNT and provides technical assistance and consultation for other health care professionals. Obtain National Provider Identification (NPI) number for reimbursement for MNT services. Other responsibilities may include assessing community nutrition needs. Develop and implement community events and programs. Collaborate with state and community agencies. Plan and deliver in-service training and education for public health personnel. Maintain quality of services in accordance with agency policies and as outlined in the Public Health Practice Reference (PHPR) and Administrative Reference (AR).

Special Requirements: Must be a licensed dietitian (LD) as provided by the Kentucky Board of Licensure and Certification as provided in KRS 310.021 or be certified as a Certified Nutritionist (CN) by the Kentucky State Board of Licensure and Certification as provided in KRS 310.031.

Starting Salary: $14.78 - $18.63 hr negotiable with additional experience. Grade 18

JOB- Health Educator I

The North Central District Health Department is accepting applications for a full time Health Educator I.

General Duties include: This position serves under the direction of the Health Education Director. Responsibilities for this position include, but are not limited to; Attend orientation sessions to develop an understanding of the role of the clinic in health assessment problems. Meet with members of the community to establish a working relationship and become familiar with available community resources. Assist in exchanging information with various organizations and agencies concerning health needs of the community and assist in assembling materials to be utilized. Gather health information for the public relative to the specialized program. Distribute health education materials by a variety of methods. Provide services according to the guidelines set by the Public Health Practice Reference/Administrative Reference (PHPR/AR). Participate as a team member, having a basic understanding of the team process. Demonstrate an awareness of public service announcements (PSA) and assist in writing public service announcements for radio and television. Submit written material to assigned supervisor for approval. Determine working contacts with radio, TV and newspaper personnel. Collect and analyze data for health program needs.

Minimum Education, Training or Experience: Bachelor Degree and one (1) year of health related or education experience.

Substitution for Education, Training or Experience: Masters Degree or Bachelor Degree in health or human services fields will substitute for one (1) year experience required. A professional license in the health or human services field will substitute for an additional year of experience.

A Registered Nurse may also substitute. Salary negotiable upon experience.

Starting Salary: $12.15 – $15.17/hr negotiable with additional experience. Grade 15
